I personally think it's the Atari version of Donkey Kong. It had only two levels, and had the worst graphics and shortcuts on sound out of any game I'd seen on the Atari.
Most people rank E.T. as the worst--I thought E.T. was actually playable.
The Swordquest series was lame primarily because they never finished the series that would have brought a glimpse of some cohesion.
